The Need
The healthcare marketplace in New England has seen rapid consolidation. There is, however, a strong faction of independent primary care practices that remain and deliver amazing value to patients. To keep this intact, our team knew we had to highlight the value (and cost savings) those independent providers provide the system. From this, Patriot Health Network was formed.

In doing so, our primary care practices are able to provide an excellent medical home, and medically manage their panel in a clinically and cost effective way.
